EyeBenefits has a nationwide provider network that includes over 12,000 optical retail stores and LASIK locations. EyeBenefits' network includes private practices, regional and national retail chain optical locations.
Headquartered in Scottsdale, Arizona, EyeBenefits (formerly First American Vision Services) has a 21 year history of discounted vision healthcare products and services. These services include offering EyeBenefits’ Members discounts on eyeglasses, contact lenses, eye examinations and LASIK procedures.
EyeBenefits is not an insurance plan; therefore there are no premiums or claims to process. The EyeBenefits’ program was originally offered to pre-paid dental plans to enhance their benefit packages. Since that time EyeBenefits has expanded its marketing efforts to HMO’s, insurance companies, third party administrators, employers and associations.
