EyeBenefits offers discounts on vision products and services to EyeBenefits' Members and their families with a network of highly qualified eye care professionals.
EyeBenefits, has a nationwide Provider Network that includes over 12,000 optical retail stores and LASIK locations. EyeBenefits' Network includes private practices, regional and national retail chain optical locations.

How does EyeBenefits' Discount Vision Program work?
There are no restrictions on style or type or limitations on quantity. Pay the discounted fee directly to the provider, as there are no forms to file or claims to submit. All Providers listed on this web site have been credentialed and contracted with EyeBenefits. If a Provider's office is uncertain if EyeBenefits is in their system, please have them call EyeBenefits directly at 800-621-7900. EyeBenefits was established in 1987 and was formerly First American Vision Services (FAVS). This is not an insurance plan.
How much can I save with EyeBenefits?
Discounts may vary due to your Provider selection of either an Independent, Regional or National store location. We suggest calling your selected Provider directly for specific discounts.

How do I find an optical location in my area?
On the EyeBenefits' website's homepage is the "Locate Provider" button. Enter both the city & state or the zip code only. (Using only the first three digits of the zip code will broaden the search area.) A list of all of the Providers in the designated area will be displayed.
How do I get my eye doctor or optical store in EyeBenefits' Provider Network?
Go to EyeBenefits’ website's homepage located at the bottom of the page is the "Contact Us" icon. Click on the icon and submit your eye doctor's name and phone number or you may nominate your eye doctor by calling EyeBenefits' provider relations department at 1-800-621-7900 ext 4.
Do you have Ophthalmologists in your Provider Network?
Ophthalmologists are medical doctors that deal with the medical functions or diseases of the eye. In most circumstances your visit would be covered under your medical insurance.
What if the optical store does not recognize or say they do not accept EyeBenefits?
All participating Providers listed on EyeBenefits’ website have been contracted and credentialed by EyeBenefits. Please have the optical store staff call EyeBenefits at 1-800-621-7900 ext. 4.
Do I get the discount if the store is running a sale?
No. However, EyeBenefits' members will always receive the lowest pricing available.
How can I buy contact lenses online?
Click on the link on the Home Page for online contact lenses. EyeBenefits' members enter your email address and proceed to find your desired contacts and go through check out. If you are an EyeBenefits' member and do not have an email address, call our Customer Care Line at 1-800-847-4663 for assistance.
How do I find a LASIK provider in my area?
Go to our "Find A Provider" and enter the state only and check the LASIK box. We suggest you call the LASIK center directly and they will assist you in finding the nearest affiliate in your area and answer any additional questions.
What are the discounts on LASIK surgery?
Your savings will be up to 50% off the national average. Please call EyeBenefits' participating LASIK center nearest you for exact discount and any other questions.
Can I use EyeBenefits' Discount Program if I have vision insurance?
Yes, EyeBenefits can be used as a supplement. For example, in some cases you are allowed to purchase one pair of eyeglasses through your insurance every year. With EyeBenefits you may purchase additional glasses or contact lenses (which insurance may not cover) at a discounted price.
How many times can I use my EyeBenefits’ membership?
There is no limit on the number of times you and your family can take advantage of the savings provided by EyeBenefits throughout the year.
How do I enroll online?
Go to www.EyeBenefits.com and click on the "Signup Online" button. Enter your group code to receive your group rate. If you don't know your group code call your organization's benefit department. Click on the "next" button and continue through the enrollment process. To complete your enrollment you will need a Visa, MasterCard or Discover credit card.
How do I Enroll online without a group code?
Same steps as above with the only exception being, leave the group code box empty and click on “next”.
I’m enrolled in the family plan. Do my family members need cards?
No. Your family members do not need their own cards to receive discounts. They just need to show your family EyeBenefits’ membership card.
How do I enroll by fax or by mail?
Call EyeBenefits' executive offices at 1-800-621-7900 and select option # 5 for your organization's enrollment form.
I lost my card. How do I replace it?
If you enrolled online:
Go to EyeBenefits’ website's homepage, click on the "Manage My Account" icon and follow the instructions. If you do not know your user name or password call our Customer Care Line at 1-800-621-7900 ext 5.
If you enrolled by mail or fax:
Send a check for $2.00 to the address located at "Contact Us" to request an additional vision card.
If you received your card through your companys' HR Department:
Contact your companys' Human Resource Director for assistance.
If you need membership verification within 24 hours:
Call our Customer Care Line at 1-800-621-7900 ext 5.
What is CRT?
Corneal Refractive Therapy ("CRT") is a non-surgical treatment that corrects myopia (nearsightedness) and astigmatism (out of round corneal surface) using the overnight wear of treatment contact lenses. The lenses gently reshape your corneas while asleep. Upon waking, and removing the lenses, you will experience clear and natural distance vision, without glasses or contact lenses throughout the day.
