How does the EyeBenefits Discount Vision Program work?

SAVING on vision care is easy, just follow these simple steps.

  • Become an EyeBenefits Member for a nominal fee; annual memberships are available for one, two, or three years.
  • Choose an EyeBenefits Optical Location.
  • Visit an EyeBenefits participating optical location. Prior to your services present your EyeBenefits Membership card and receive your discounts.

With your EyeBenefits Membership there are no limitations on style, type or quantity of products and services. After your discount has been applied, pay the remaining balance directly to the EyeBenefits optical location; there are no forms or claims to submit. This is not an insurance plan.

In addition to the chart above Wal*Mart Vision Center (Corporate Operated) will be offering 5% off eye glasses only. The Vision Center in Wal*Mart will be offering 20% off eye glasses. All optical locations listed on this website have been credentialed and contracted with EyeBenefits. If an optical location is uncertain if EyeBenefits is in their system, please have them call EyeBenefits directly at 800-621-7900.

How often can I use my EyeBenefits Membership?

You and your family have the opportunity to utilize the EyeBenefits Vision Discount Program as many times as you feel necessary. There are no limits to the number of eye exams, eyewear and contact lenses that you can purchase throughout your EyeBenefits Membership. 

How do I enroll online?

Visit the EyeBenefits website homepage and click on the "Sign Up Online" tab. Enter your group code to receive your group rate. If you don't know you group code call your organization's benefit department. Proceed by clicking "Next" and continue through the enrollment process. To complete your enrollment you will need a Visa, MasterCard, Discover, American Express or debit card. 

How do I enroll online without a group code?

Visit the EyeBenefits website homepage and click on the "Sign Up Online" tab.Leave the group code field empty and click "Next". To complete your enrollment you will need a Visa, MasterCard, Discover, American Express or debit card. 

How do I enroll by fax or by mail?
Call the EyeBenefits Executive Offices at 800-621-7900 and select option 5 and request your organization's enrollment form. 
I am enrolled in the family plan. Do my family members need their own membership cards?

Your family members do not need their own Membership Cards to receive the discounts. However, you may purchase additional membership cards for $2.00 each. 

Can I receive a cancellation refund?

All cancellation refunds will be given no later than 30 days after the Member receives their EyeBenefits Membership Card. For questions, concerns or complaints please call Customer Care at 800-621-7900. All inquires will be acknowledged in writing within 5 business days and the results of the investigation will be provided no later than 30 days after the complaint is received.

I lost my EyeBenefits Membership Card. How can I replace it?

If you enrolled online:
          Visit the EyeBenefits website homepage and click on the "Manage My Account" tab. You will then be prompted to 
          enter in your Username and Password. If you do not know your Username or Password call EyeBenefits Customer
          Care at 800-621-7900 ext 5.

If you enrolled my mail or fax:
          Mail a check for $2.00 to 
          PO Box 14106
          Scottsdale, AZ 85267

If you received your Membership Card through your company's Human Resource Department:
          Please contact your Human Resource Director for further assistance.

Can I use my EyeBenefits Discount Vision Program if I currently have vision insurance?

Yes, EyeBenefits may be used with your current insurance plan. In some cases you may only purchase one pair of eyeglasses through your insurance plan eyewear year. With EyeBenefits, there are no limits to the number of eye exams, eyewear and contact lenses that you can purchase throughout your Membership. 

How do I find an optical location in my area?

Visit the EyeBenefits website homepage and click on the "Optical Location" tab.Enter both the city and state or the zip code, using only the first three digits of the zip code will broaden the search area. A list of all the optical locations will be displayed. 

What if the optical location does not recognize or say they do not accept the EyeBenefits Discount Vision Program?

All optical locations listed on the EyeBenefits website have been contracted and credentialed with EyeBenefits. If an optical location is uncertain if EyeBenefits is in their system, please have them call EyeBenefits directly at 800-621-7900. 

How can my eye doctor or optical location join in the EyeBenefits Provider Network?

Visit the EyeBenefits website homepage and click on the "Become A Provider" tab. Complete the information fields or you may nominate your eye doctor by calling EyeBenefits Provider Relations Department at 800-621-7900 ext 4. 

Will I receive my EyeBenefits discount if the optical location is having a sale?

EyeBenefits Members will receive their EyeBenefits vision discount, however it is the optical locations decision to combine the discounts

How can I buy contact lenses online?

Visit the EyeBenefits website homepage and click on the "Online Contact Lens" tab. EyeBenefits Members will then be prompted to enter their email address. If you are an EyeBenefits Member and do not have an email address, please call Customer Care at 800-621-7900 for assistance. 

What is an Ophthalmologist?
Ophthalmologists are medical doctors that work with the medical functions or diseases of the eye.
Do You have Ophthalmologists in the EyeBenefits Provider Network?
Yes, EyeBenefits currently has over 1,500 LASIK locations nationwide. 
How do I find a LASIK location in my area?

Visit the EyeBenefits website homepage and click on the "Optical Locations" tab. Enter both the city and state or the zip code, using only the first three digits of the zip code will broaden the search area. Next, check the "LASIK Only" box and submit. a list of all LASIK locations will be displayed.
