How does EyeBenefits Discount Vision Program work?
- Purchase an EyeBenefits Membership for a nominal fee, which are available
for one, two, or three years.
- Go to an EyeBenefits participating optical location. Prior to purchase, present your
EyeBenefits Membership Card and receive your discounts.
- For LASIK procedures, schedule an appointment with a participating
EyeBenefits' physician in our national network.
With your EyeBenefits Membership there are no limitations on style, type or quantity
of products and services. After your discount has been applied, pay the remaining
balance directly to the EyeBenefits optical location; there are no forms or claims to
submit. All optical locations listed on the EyeBenefits website have been credentialed and
contracted with EyeBenefits. If an optical location is uncertain if EyeBenefits is in
their system, please have them call EyeBenefits directly at 800-621-7900.

How do I find an optical location in my area? Visit the EyeBenefits website homepage and click on the "Optical Location" tab. Enter both the city and state or the zip code, using only the first three digits of the zip code will broaden the search area. A list of all of the optical locations will be displayed.
How can my eye doctor or optical location join in the EyeBenefits Provider Network? Visit the EyeBenefits website homepage and click on the "Become a Provider" tab. Complete the information fields or you may nominate your eye doctor by calling EyeBenefits Provider Relations Department at 800-621-7900 ext 4.
What is an Ophthalmologist? Ophthalmologists are medical doctors that deal with the medical functions or diseases of the eye.
Do you have Ophthalmologists in the EyeBenefits Provider Network? Yes, EyeBenefits currently has over 900 LASIK locations nationwide.
What if the optical locations do not recognize or say they do not accept the EyeBenefits Discount Vision Program? All optical locations listed on the EyeBenefits website have been credentialed and contracted with EyeBenefits. If an optical location is uncertain if EyeBenefits is in their system, please have them call EyeBenefits directly at 800-621-7900.
Will I receive my EyeBenefits discount if the optical location is having a sale? EyeBenefits Members will receive their EyeBenefits vision discount, however it is the optical locations decision to combine the discounts.
How can I buy contact lenses online? Visit the EyeBenefits website homepage and click on the "Online Contact Lens" tab. EyeBenefits Members will then be prompted to enter their email address. If you are an EyeBenefits Member and do not have an email address, please call Customer Care at 800-847-4663 for assistance.

Can I use the EyeBenefits Discount Vision Program if I currently have vision insurance? Yes, EyeBenefits may be used with your current insurance plan. In some cases you may only purchase one pair of eyeglasses through your insurance plan every year. With EyeBenefits, there are no limits to the number of eye exams, eyewear and contact lenses that you can purchase throughout your Membership.
How often can I use my EyeBenefits Membership? You and your family have the opportunity to utilize the EyeBenefits Vision Discount Program as many times as you feel necessary. How do I enroll online?
Visit the EyeBenefits website homepage and click on the "Sign Up Online" tab. Enter your group code to receive your group rate. If you don't know your group code call your organization's benefit department. Proceed by clicking "Next" and continue through the enrollment process. To complete your enrollment you will need a Visa, MasterCard, Discover, American Express or debit card.
How do I Enroll online without a group code? Visit the EyeBenefits website homepage and click on the "Sign Up Online" tab. Leave the group code field empty and click "Next". To complete your enrollment you will need a Visa, MasterCard, Discover, American Express or debit card.
How do I enroll by fax or by mail? Call the EyeBenefits Executive Offices at 800-621-7900 and select option 5 and request your organization's enrollment form.
I am enrolled in the family plan. Do my family members need their own membership cards? Your family members do not need their own Membership cards to receive discounts. However, you may purchase additional membership cards for $2.00 each.
I lost my card. How do I replace it?
If you enrolled online: Visit the EyeBenefits website homepage and click on the "Manage My
Account" tab. You will then be prompted to enter in your Username and
Password. If you do not know your Username or Password call
