How often can I use my EyeBenefits Membership?

You and your family have the opportunity to utilize the EyeBenefits Vision Discount Program as many times as you feel necessary. There are no limits to the number of eye exams, eyewear and contact lenses that you can purchase throughout your EyeBenefits Membership. A membership is active for one year.

How do I enroll online?

Visit the EyeBenefits website homepage and click on the “Become a Member” menu item. Enter your group code to receive your group rate. If you don’t know you group code call your organization’s benefit department. Proceed by clicking “Next” and continue through the enrollment process. To complete your enrollment you will need a Visa, MasterCard, Discover, American Express or debit card.

I am enrolled in the family plan. Do my family members need their own membership cards?

Your family members do not need their own Membership E-Cards to receive the discounts.

Can I receive a cancellation refund?

All cancellation refunds will be given only if an EyeBenefits Member hasn’t downloaded their membership e-card. If a member has purchased from a third party organization they need to contact that organization directly. For questions, please call Customer Care at 1-513-201-3037.

I lost my EyeBenefits Membership E-card. How can I replace it?

If you enrolled online: Visit the EyeBenefits website homepage and click on the “My Account” tab. You will then be prompted to enter in your Username and Password. If you do not know your Username or Password call EyeBenefits Customer Care at 1-513-201-3037.

If you received your Membership E-Card through your company’s Human Resource Department: Please contact your Human Resource Director for further assistance. Or if you purchased your membership from another organization.

Can I use the EyeBenefits Discount Vision Program if I currently have vision insurance?

Maybe. EyeBenefits may be used with your current insurance plan if the Provider you have selected allows it. Please check with your Provider when you call for an appointment.

How do I find an EyeBenefits provider in my area?

Visit the EyeBenefits website homepage and click on the “Find an EyeBenefits Provider” tab. Enter your city, state or zip code. A list of all the our providers be displayed.

What if my location does not recognize or accept the EyeBenefits Vision Discount Program?

All providers listed on the EyeBenefits website have been contracted and credentialed with EyeBenefits. How can my eye doctor join the EyeBenefits Provider Network?

Visit the EyeBenefits website homepage and click on “Providers” in the menu and complete the information fields.

Will I receive an EyeBenefits discount if my provider is already having a sale?

EyeBenefits Members will receive the better of their EyeBenefits vision discount or the sales price, whichever is the better discount, however it is the optical locations decision to combine the discounts.
